Acts to be often made, but especially in the time of fickness and before death, by him, who defires to live and die as a Chriftian. IDO, with all humility and obedience of Mind, accept from the Divine Providence, this, and any other cross, disease, and even death itself; intirely fubmitting my will, to the holy Will of God; and acknowledging myself, not only most worthy of this affliction, disease and death, but of any other more painful and fevere punishment, (may I not fay i even Hell itself) for my innumerable offences.

Moreover, Iaccept this crofs, infirmity and death, in reverence of the most bitter Paffion of our Lord Jefus Chrift; uniting the pains, troubles and griefs that I undergo, to the bitter torments, which He fuffered in His Life Paffion and Death; and moft humbly befeechIng Him to accept the fame, for all the Pains! ought to fuffer in the next life, for my innumerable fins.

2. As by the Grace of God, I now am in the true Faith of the Holy, Catholic and Apoftolic Church; and do moft firmly believe whatever the Church doth believe, and hath proposed to be believed; and in particular the Articles of the Holy Creed; fo I defire and refolve to die in the fame Faith; and therefore confeffing the fame I fay, I BELIEVE in God the Father Almighty, Maker of Heaven and Earth; and in Jefus Chrift his only Son our Lord; who was conceived by the Holy Ghoft, born of the Virgin Mary: fuffered under Pontius Pilate; was crucified, dead and buried; He defcended into Hell; the third day he arose again, from the Dead; He afcended into Heaven, and fits at the Right Hand of God the Father Almighty; from thence he fhall come to judge the living and the dead. I believe in the Holy Ghoft; the Holy Catholic Church; the Communion of Saints; the Forgiveness of Sins; the Re

furrection of the Flesh; and Life everlasting. Amen. And I understand all thefe Things, in the fame fenfe, that our Holy Mother the Church hath always understood them.

3. I humbly afk pardon for all my fins committed against the Divine Majesty, my neighbour, and myfelf, in thought, word and deed, in the omiffon of the observance of the Commandments of God, of the Precepts of his Church, and of Works of Mercy, whether spiritual or temporal; and in the abuse of the Holy Sacraments; in the negligent guard of my fenfes: And for my fecret fins, and the fins, that I have caused in others. I humbly afk God forgivenefs, thro' his infinite goodness and mercy, thro' the merits of the life, paffion and death of my Lord, and his Son, Jefus Chrift; and by the powerful Interceffion of the glorious Virgin Mary, the Mother of Chrift; and of all the Holy Angels and Saints.

4. I do, from my very heart, forgive all perfons, who at any time, or in any manner, have injured or offended me; and I beg of the Divine Majefly for them, all those bleffings, which I defire for my own foul; and I do alfo defire pardon, if I have given them occafion to offend me; and I befeech the Omnipotent God, that I may profoundly and lively have this fentiment in my heart; that
